GUIDELINES FOR SUPER FUND REQUESTS

SCROLL DOWN FOR SUPER FUND STATUS REPORT

 

Starting January 1, 2003, to qualify for Super Fund Monies, your chapter must have hosted a Super Fund/Membership Banquet within the last 12 months.

 

The following required information must be supplied on your Super Fund requests in order to be considered for approval.  Besides the appropriate code numbers, the more information you can supply to each Super Fund request will insure quicker consideration and, if approved, a faster check return.

 

ALL SUPER FUND REQUESTS MUST BE MAILED TO THE KENTUCKY STATE SECRETARY FOR PROPER HANDLING, PROCESSING AND TRACKING!

 

JAKES DAY:  Scheduled date, location of the event and planned activities.

 

WOMEN IN THE OUTDOORS:  All events must be scheduled with the Women's Regional Coordinator to qualify for Super Fund dollars.  The Women's Regional Coordinator will write all Super Fund requests for this event.

 

WHEELIN SPORTSMEN:  All events must be scheduled with the Wheelin' Sportsmen's Regional Coordinator. 

 

SCHOLARSHIPS:  Recipient's complete address/phone number and the entire scholarship information packet must accompany all Super Fund requests for scholarships.

 

ADDITIONAL EDUCATION BOXES:  Supply the number of additional boxes needed and the names of the teachers and schools who will receive these boxes.

 

4-H SHOOTING SPORTS:  Chapter must supply an explanation as to what this money is to be used for, such as: meet registration fees, travel expenses (school buses), designated shooting area expenses, and needed equipment.  (Please specify what equipment, supplies and/or materials your chapter is intending to purchase with Super Fund money).   A letter from the 4-H County Extension Agent in charge of the 4-H Shooting Sports programs, in the county or counties where this money is to be spent must accompany all 4-H Super Fund requests.  This letter should include the names of coaches and an explanation of how this money will be used for this program.  All new shooting programs should include an action plan that illustrates the program, instructors, training plan and goals of the program.  This action plan must be submitted with the Super Fund request by the chapter, along with an explanation of active involvement by the County Agent for 4-H/Youth Development and the level of financial support from the County Extension Office for the 4-H Shooting Sports Program.

 

ARCHERY IN THE SCHOOLS PROGRAM:  Same as 4-H requests, with the exception that the letter needs to come from the school principal, where this program has been installed, with all the necessary information required.

 

FFA:  Same as 4-H requests, with the exception that the letter needs to come from the FFA coach supplying all the necessary information required.

 

HABITAT ENHANCEMENT & MANAGEMENT:  Specific information explaining what the project entails and how the money will be spent, list all cooperatives/partners and their donations, total acreage involved, a list of items or equipment to be purchased, and where this equipment will be stored and who will have access to it.

The above information must be submitted along with Super Fund requests covering habitat equipment, prescribed burns, payment for standing crops, contract labor, equipment rental, wildlife openings, water development, signs, habitat enhancement, Riparian restoration, tree plantings, winter assistance, timber stand improvement, invasive/exotic control, supplement cost share program, private lands project through established Super Fund programs, regional program support, signs, spring seed subsidy, fall seed subsidy, Project Help, and conservation seed program.  (All 5000 codes)  All Super Fund requests for spring and fall seed subsidy must have the original or a copy of the paid seed receipt.  For chapters that require the $400.00 first to purchase  seed, an invoice showing what you types of seed you will be purchasing, the quantity, total dollars, and store name must accompany your Super fund request.  Within 30 days after receiving your Super Fund check, your paid receipt must be mailed in to the State Secretary.
